I started out using the Contact Form 7 plugin on my sites and my client sites. It is great for simple forms, but you really need to know html code in order to set up a more in depth form or questionnaire. It wound up becoming annoying.
A client of mine told me about Gravity Forms and I have been using it on my site for the past year or so. I am thrilled with the results. You can see examples of several of the forms and questionnaires that I use on my “Contact Page“.

The Gravity Forms plugin is one of the best options when it comes to website forms and questionnaires for a WordPress website or blog. I highly recommend it. It currently costs only $39 for one website, but you can have multiple forms on that one website. They also have a Business Plan for $99 where you can have up to 3 websites using the plugin. I would definitely check it out. It will definitely add value and functionality to your site.
